As much as I would like to, I have not.... either as an installer or wearer ( I don't have the right parts :D ).

Seriously though, it's a rather ingenious device in that the user can put different length screws into the cups depending on how severe you want the torture to be. I'm familiar with screws of that size (approx. 3mm), and I wouldn't advise very deep penetration (in real life) with that many screws. Blood would indeed flow.
But I would think it would quite painful if pressed into the breast just short of breaking the skin.

I have a bra with tacks, it's reasonable in length, and very exciting to wear (for a masochist of course)

Of course.
I would think a tack lined bra that's worn under normal street clothes would be much more exciting. A constant dose of pain and pleasure while you go about your daily activities. That device I posted would be better suited to the soundproofed basement playroom and a long afternoon session of suffering.
