I agree with Monty, it's a spectacular picture. Don't want to play the tedious art critic here, but that image has it all: composition, balance, dynamism, well drawn anatomy, it tells a story.
The thumbnail captures the eye even if it's buried among many others, and this is almost always a great indicator of a well done artwork.

Nicely presented for the whip, perhaps something similar to the scene in the movie Poor Cecily. All females are created "equal" when they're stretched this tightly.

The 'clack....clack...clack' of the wheel turning would be pschological torture before the pain of having your genitals stretched like that. And then you can only wonder what else awaits, especially if the torturer is a particularly one...
