As usual I come late to this intriguing thread... 
Well, just like many have already said, at a first glance, raising the patibulum up a stipe permanently fixed in the ground seems more practical and, therefore, more realistic. I think this method was really used many times in the past.
On the other hand, nailing wrists and feet of the victim to the cross when it is still laying on the ground, I think is very erotic. There is this powerful image of the executioners circling like a pack of wolves their prey and inflicting her horrible tortures. Morever, rising the cross with the victim fully nailed to it is quite hot too, and the scene where the stipe slides down the hole and when it hits the bottom producing other distrissing screams from the condemned mixed to the dull sounds of the wedges being hammered in place is very powerful too. I guess that, given its spectacular nature, this method was used too, maybe for specif executions.
So, given that the question is not which method is more realistic, but which is more effective from the visual point of view, I vote for nailing hands and feet first, then hoisting the cross after .
