As for the mea culpa, the current form in the English language service is in fact "through my fault, through my fault, through my most grievous fault". It is apparently in the ablative (how's my grammar? She's doing fine

I am an Anglican liturgist and would entirely agree that a sense of style in the writing has been sacrificed at the altar of theological correctness.
